what seems to be the problem at the moment?
　　你能告诉我，…，你怎么了?
　　i seem to be getting very short of breath nowadays, so i thought it best to come in to see you. i also feel a tightness in my chest.
　　最近我似乎总感到非常气急，胸口发紧，所以我想应该去看看医生。
　　how long has this been going on?
　　这样有多久了?
　　it seems to have been getting worse over the last few weeks.
　　好像是从上几周开始加重了。
　　are you short of breath all the time, or is it just when you walk up the stair?
　　是总感到气急呢，还是只是上楼梯的时候气急?
　　well, it's hard work going up the stairs. the pain feels like a knife.
　　上楼梯的时候疼痛会加重，就像刀割一样。
　　does the pain move to other areas as well?
　　疼痛向其他部位放射吗?
　　yes, it moves to my neck, my left arm, my back and my shoulder.
　　向我的颈部，左臂，肩背部放射。
　　do you ever feel sick or sweaty when that happens?
　　疼的时候感到恶心或者出汗吗?
　　well, i don't feel sick but i often break out in a sweat.
　　嗯，不恶心，但是很能出汗。
　　do you ever feel that your heart beats unusually fast or slow?
　　有没有感到心跳不寻常地加快或者减慢?
　　yes, sometimes it beats really slowly. then from time to time it skips a beat, then continues tobeat very rapidly.
　　有时心跳得很慢。有时心(比正常节律)少跳一下，紧接着跳得非常快。
　　can you tap out with a finger what the rhythm feels like to you? good. have you had anyswelling in your ankles?
　　你能用手指敲出节奏像什么吗?很好。你的脚踝肿胀吗?
　　yes, both of my feet have been somewhat swollen.
　　嗯，两只脚的脚踝都有点肿胀。
　　does it go down overnight or are they still swollen in the morning?
　　过夜后肿胀消失吗，还是早上的时候脚踝依然肿胀?
　　oh, my feet feel much better, and they aren't as swollen in the morning.

心脏问题
　have you ever had any heart problems?

　　i've never been diagnosed with anything but i think something could be wrong with my heart.
　　从来没有被诊断过有什么问题，但是我觉得我的心脏可能有点问题。
　　what seems to be the problem with your heart?
　　你的心脏怎么了?
　　it beats irregularly especially when i smoke.
　　心跳不规律，尤其是当我吸烟的时候。
　　what kind of pain do you get during exercise?
　　锻炼的时候你感到哪种类型的疼痛?
　　i get pressure in my chest sometimes when i have to walk a long distance.

　　absolutely. i'm always short of breath when i go upstairs.

　　do you ever feel sweaty when this happens?

　　yes, i break out in a really bad sweat when my chest hurts.

　　yes, my ankles seem to be swollen all the time.

　　yes, the pain is fairly constant, but it helps when i put my feet up.

　　yes, i eat quite a bit more when i feel good.

so, you say that you are having problems with bleeding or bruising.

　　你说你有出血和淤青的问题。

　　yes, i am.

　　是的。

　　can you tell me what you mean by that? do you bleed or bruise easily?

　　你能告诉我那是什么意思吗?你很容易出血或者有淤青?

　　yes, every time i scrape myself or bump into something i get bruised.

　　have you always had this problem, or is it just a recent development?

　　i noticed that the problems started about the time i started shaving.

　　which is more difficult, breathing in or breathing out?

　　breathing out is harder.

　　i'm short of breath when i'm working.

　do you have regular bowel movements?

　　你的排便规律吗?

　　no, sometimes i get constipated and sometimes i have diarrhea.

　　不规律，有时我便秘，有时我腹泻。

　　when you say you have diarrhea, do you mean the stool is very loose, or that you have to go to the toilet more frequently?

　　你说你腹泻，是指大便稀，得频繁上厕所?

　　i have to go four or five times a day.

　　我一天去四、五次。

　　when you have to go, do you have to go quickly?

　　当你不得不去的时候，很急吗?

　　yes, i have real problems holding it.

　　是的，我总是憋不住。

　　could you describe your stool?

　　the last time it was loose and watery, but very dark.
